Bob was an Emergency Medical Hologram Mark I that was programmed by Starfleet in the early 2370s. (Star Trek: Intrepid)

Origins

In 2378, Bob was accidentally installed aboard the USS Intrepid-A instead of a Mark III, and was immediately replaced when the flaw was discovered. When the Intrepid crew found out it would be sent to mine dilithium along with hundreds of other Mark I's, some of the crew demanded that the Mark I be reinstalled and allowed to fulfill his designated purpose. From that point on, the Mark I functioned as an excellent back-up to Lowes's medical team, and Doctor Lowes would often assign him to research projects. As the Mark I became a functioning member of the Intrepid's crew, he decided that he needed a name, although he couldn't initially think of one. He eventually chose the name "Bob". (Star Trek: Intrepid)

Adaptations

After reading about the exploits of the USS Voyager's EMH in the Delta Quadrant, Bob decided that he wanted to have more freedom and responsibilities aboard ship. Science officer Kell Perim and Chief Engineer B'Elanna Torres worked on expanding his programming so that he could function as an engineer and bridge officer. Unfortunately, the project hit several snags when the new protocols and subroutines were introduced into Bob's programming, including near fatal degradation in his program. It was later repaired by B'Elanna Torres. An ability unique to The Doctor, Voyager's EMH was able to leave sickbay or the holodeck and to roam freely with the help of a mobile emitter (originating from 29th century technology), in 2378 the USS Intrepid-A found another mobile emitter originating from 29th century technology in space and Bob was given the mobile emitter and was able to leave sickbay or the holodeck and to roam freely on the Intrepid.
